Problem and difficult decision making/ 問題と難しい決断

As we went to the appointment today, the doctor told us very surprisingly that there is a problem now for Sachi's treatments. For the study protocol, it must show that a tumor in the breast is also Her2 positive. The last biopsy only showed that the cancer in the lymph node is Her2 positive but not in the breast. The doctor thinks that there is a Her2 tumor somewhere in the breast but the last biopsy couldn't detect it. So, Sachi's been told to do the biopsy again. Otherwise, we can choose the conventional chemotherapy that can start sooner.  

For this new biopsy, a specialist will perform it but we must wait for some time until he's available. It can be next week but it can also be a few weeks later. Then from the time of biopsy, we will need to wait for about 10 days for the result. The study protocol would only then start (About two to three or more weeks of waiting from the biopsy). Furthermore, if they can't detect the Her 2 positive tumor in the breast again, Sachi will have to do the conventional treatments with chemotherapy, not the study protocol, despite all the waiting time. 

This Thursday, the day after tomorrow, we have been booked to speak with the doctor again. He will tell us when the biopsy is and we will discuss if we want to still participate in the study protocol or change to the conventional therapy. 

Up until now, plans have changed several times at the hospital and honestly speaking, we are quite tired from leaving the old and adjusting to the new, and experiencing something new again.

There are advantages and disadvantage in both the study protcol and the conventional therapy. Please pray for God's wisdom and leading in our decision making. It's a big and difficult decision to make and we have only one and half day for it. Thank you for praying.

今日病院の予約に行くと、予想もしていませんでしたが、医師はサチの治療を進める上で問題があると言いました。研究プロトコルでは、乳房の腫瘍もHer2陽性でなければならないそうなのですが、前回の生検では、リンパ節の腫瘍はHer2陽性であったものの、乳房の腫瘍の生検はHer2を示さなかったのです。乳房のどこかにHer2腫瘍があると医師は考えていますが、前回の生検ではそれを検出できませんでした。そして、サチは再び生検をするように言われました。そうでなければ、より早く開始できる標準療法の抗がん剤を選択できます。

新たに生検する場合は、スペシャリストが行いますが、しばらく待つ必要があります。来週予約を取れるかもしれませんが、何週間も後になる可能性もあります。そして、生検から結果が出るまで約10日待つ必要があります。そしてようやく研究プロトコルが開始されます（生検から約2-3週間以上かかるでしょう）。さらに、乳房のHer 2陽性腫瘍を再び検出できない場合、サチは、どれだけ長く待ったにも関わらず、研究プロトコルではなく、従来の標準治療を行うことになります。

明後日の木曜日、再び医者と話します。彼はその時生検がいつになるかを教えてくれ、私たちがまだ研究プロトコルに参加したいのか、それとも従来の標準治療に変更したいのか話し合います。

今までも病院では、何度かプランの変更があり、正直に言えば、夫婦共々計画変更に振り回されて疲れています。提示された計画にようやく心が決まったと思ったら、また計画が変わりそれに順応します。するとまた新たな変更がある。

研究プロトコルと従来の標準治療の両方に長所と短所があります。私たちが治療法を決断する上に、神さまが知恵と導きをもって望んでくださるようお祈りお願いします。難しくて大きな決断で、決断までに1日半しかありません。よろしくお願いします。
